Most Medicare members use only a small slice of what Medicare actually covers. The rest sits on the table. Some of these benefits are free. Some come with most Medicare Advantage plans. Most people never even know they exist. Here are 10 of the most overlooked.
1. Free Annual Wellness Visit
Every year, Medicare gives you a free wellness visit. This is not the same as a yearly physical. It is a chance to update your health history, set a prevention plan, and talk through any concerns. There is no copay, no deductible, no coinsurance.
Most members skip this visit because they think they already had a physical. The two are different. Ask your doctor for the "Annual Wellness Visit" by name to make sure you get the free one.
2. Free Cancer Screenings
Medicare covers a long list of cancer screenings at 100%. That means $0 out of pocket. The list includes:
- Mammograms (every 12 months for women 40 and older)
- Colonoscopy (every 10 years, or more often if you are higher risk)
- Cervical cancer screening (every 24 months)
- Prostate cancer screening (yearly for men 50 and older)
- Lung cancer screening (yearly for certain current and former smokers)
You may still owe a small copay if the test turns into a procedure (like a colonoscopy that removes a polyp). But the screening itself is free.
3. Free Vaccines
Medicare Part B and Part D together cover most vaccines you need as a senior at no cost:
- Flu shot (yearly)
- COVID-19 vaccine
- Pneumonia vaccines
- Shingles (RZV) vaccine
- RSV vaccine (for adults 60 and older)
- Hepatitis B (for higher-risk patients)
The shingles and RSV vaccines used to be very expensive. As of 2023, Part D covers them with $0 cost share. Worth grabbing both if you have not.
4. Diabetes Prevention Program
If you are at risk for type 2 diabetes (based on your blood sugar and weight), Medicare covers a 12-month diabetes prevention program for free. It is a structured program with health coaching, group sessions, and a long-term plan to keep diabetes away.
This benefit is once in a lifetime, but most people who qualify do not even know it exists.
5. Smoking Cessation Counseling
Medicare covers up to 8 counseling sessions per year to help you quit smoking. There is no copay. Some medications are also covered under Part D. If you are trying to quit, this is real money on the table.
6. Mental Health Screenings and Counseling
Medicare covers a yearly depression screening at no cost. It also covers therapy and counseling visits with a licensed mental health provider, with a small copay. As of 2024, marriage and family therapists and licensed counselors are now Medicare providers, which opens up many more options.

7. Diabetic Supplies and Continuous Glucose Monitors
Medicare covers blood sugar test strips, lancets, glucose meters, and even continuous glucose monitors (CGMs) for people who use insulin or have a doctor's prescription. The CGM benefit alone has saved seniors thousands of dollars per year.
As of 2023, insulin copays are capped at $35 per month per type. As of 2026, the entire Part D out-of-pocket cap is $2,100, which protects diabetics from runaway drug costs.
8. SilverSneakers and Gym Memberships
Most Medicare Advantage plans include free access to fitness programs like SilverSneakers, Renew Active, or Silver and Fit. That means a free membership at thousands of participating gyms, plus online classes and senior-friendly workouts. Original Medicare does not include this. Many people switch to Medicare Advantage just for this benefit and do not realize what they give up elsewhere.
9. Over-the-Counter (OTC) Allowance on Medicare Advantage
Many Medicare Advantage plans give you a monthly or quarterly allowance for over-the-counter products. The amount ranges from $25 to $200 a month. You can use it at certain stores or order from a catalog. Common items include pain relievers, vitamins, first aid supplies, and sometimes groceries.
This benefit varies a lot between plans, so check your plan's Evidence of Coverage to see what yours offers.
10. Foot Care for Diabetics
If you have diabetes-related nerve damage or circulation issues in your feet, Medicare covers a yearly foot exam plus therapeutic shoes or inserts. That is a benefit worth $200 to $500 a year that almost no one uses.
The bottom line
Medicare is not just hospital insurance. It is a long list of benefits, screenings, and supports that you already pay for through your premium. Most members use about 20% of what is available. Spend an hour with your benefits booklet (or with an independent agent) and you may find $1,000 or more of benefits sitting unused.
